Account Setup

Our account setup form is now available for download. Page 1 is the actual application and Page 2 must be filled out in order to authorize Napoli Foods to view past credit history. Use Page 3 to instruct us on where and when deliveries should be made. Standard credit terms are 7 days from date of purchase. Please note that we can only accept credit applications in writing. Be sure to contact us with any questions.

Credit Form

For your convenience we have provided a copy of the Connecticut Department of Revenue's Sales & Use Tax Resale Certificate, which is a required part of the credit application. Page 1 of the document outlines the state's regulations for resale certificates for sales tax exemption (Regulation 1) and the required record-keeping practices (Regulation 23). Page 2 is the actual certificate and may be filled out on the computer prior to printing.
